Job Summary:
The location is Martinsville, Indiana. The healthcare organization is seeking a licensed Outpatient Physical Therapist (PT) to provide professional therapy services. The selected candidate will evaluate patients, develop and implement treatment plans, and document patient progress in accordance with regulatory and organizational standards. Candidates are required to bring their license on the first day of the assignment.

Job Details:

• Address: 35 BILLS BOULEVARD, MARTINSVILLE, INDIANA, 46151
• Schedule: Days shift, 8 hours daily, minimum of 40 hours per week
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 1+ Year of Outpatient Clinical Experience; CPR Certification; active/applicable license for the State where work will be performed
• Preferred Qualifications: Experience with previous assignments with the agency or organization is highly desired; experience with outpatient therapy preferred
• Additional Information: Completes comprehensive patient evaluations, adheres to state practice acts and professional standards, participates in student teaching, maintains licensure and certifications, and contributes to organizational performance improvement initiatives.
