Job Summary:
The organization is seeking a Physical Therapist specializing in Outpatient services in Sherwood, Oregon. This position involves providing outpatient physical therapy care, with an emphasis on documentation and compliance with organizational and regulatory standards. Candidates must bring their active license on the first day of the assignment.

Job Details:

• Location: Sherwood, Oregon
• Specialty: Physical Therapist Outpatient (PT OP)
• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week
• Required Qualifications: Active physical therapy license. Previous experience and any necessary credentials must be verified. Candidates must have the appropriate license, with a requirement for a permanent license for SC (state-specific). No compact licenses accepted.
• Additional Information: Candidates are required to complete some documentation directly with the organization outside of the organizations clearance process before beginning the assignment. Refunds for holiday weeks are included as part of the operational protocol. Candidates with prior work history at the organization as a permanent or PRN employee within the last year are not eligible to rehire until separation of at least one year. Those with previous travel or perm/PRN work more than a year ago should send their name and date of birth to the designated contact for eligibility review.

• Bill GWW (Gross Work Hours): 40 hours
• Pay GWW: Yes
• OT Bill: 1.5 times the regular rate for overtime hours
• Notice: 30 days
• Parking: Will be provided during the first day
• Dress Code: Will be provided during the first day
