Job Summary:
The facility is seeking a qualified Physical Therapist (PT) specializing in Home Health services to serve in the Portland, Oregon area. This position involves providing patient-centered care in a home health setting, with an emphasis on rehabilitation and functional recovery. The start date is anticipated to be August 10, 2026, and the assignment is structured for 8-hour days.
Job Details:
- Location: Portland, Oregon (zip code 97225)
- Position: Physical Therapist (PT) Home Health
- Schedule: 8 hours per day, minimum of 40 hours per week, start date 08/10/2026
- Required Qualifications: Valid physical therapy license, at least 1 year of home health experience, BCLS certification, reliable transportation, and the ability to work in a home health setting.
- Open to First-Timers: Yes
- Certification Required: BCLS (Basic Cardiac Life Support)
- Experience Needed: 1 year of skilled home health experience
- Patient Ratio Experience: Not specified
- Charting System Experience: Required, with familiarity in community-based hospital settings preferred
- Special Equipment/System Requirements: Proficiency with Electronic Medical Records (EMR) systems such as EPIC and Rover for charting and documentation
- Travel: Occasional travel within the Portland metropolitan area, approximately 60 miles maximum daily, with good routing to minimize drive time
- Notes: The traveler is expected to have their own transportation. The facility can accommodate a block schedule and will consider local candidates within a 50-75 mile radius.
- Additional Requirements: References covering 1 year (6 months for CNAs) are required. All travelers may be expected to float between facilities within the same state.
