Job Summary:
Physical Therapist Home Health in Marlborough, Massachusetts. This temporary assignment involves providing skilled physical therapy services to patients in a home health setting. The role requires adherence to state-specific licensing and supervision requirements, including developing treatment plans, evaluating patient progress, and coordinating discharge planning.

Job Details:

• Schedule: Days shift, 8 hours per day, Monday through Friday, with a minimum of 40 hours per week.
• Required Qualifications: Current and unencumbered license to practice physical therapy in Massachusetts; completion of a physical therapy program approved by the American Physical Therapy Association or equivalent; 3 years of physical therapy experience; current CPR certification; proficient in general computer use including internet, email, and navigation of computer systems.
• Preferred Qualifications: Experience in home health therapy settings preferred.
• Additional Information: The physical therapist will perform duties including providing treatment as ordered by physicians, assisting in evaluating patients functional levels, developing and updating care plans, and participating in in-service education programs. The assignment will involve traveling approximately 100-150 miles weekly between assigned territories, utilizing HCHB electronic medical records, and supervising physical therapist assistants as appropriate. Reliable transportation, current drivers license, and agency-required liability insurance are necessary.
