Job Summary:
The organization is seeking a Physical Therapist specializing in Home Health services in Clear Lake, California. The role involves planning and conducting treatment programs aimed at restoring and improving patients' physical, social, and mental functions. Candidates are required to bring their license on the first day of employment.

Job Details:

• Location: Clear Lake, California
• Schedule: Day Shift, 8 hours per day, Monday through Friday, no weekends, minimum of 40 hours per week
• Required Qualifications: Doctor of Physical Therapy (DPT) degree obtained on or after January 2016, or foreign equivalent; valid Physical Therapy licensure in all applicable states of practice; current AHA-BLS or AHA-CPR certification; valid Drivers License and being at least 21 years of age or older; at least 1 year of home health experience; proficiency with HCHB and OASIS electronic medical record systems
• Additional Information: Must have knowledge of HCHB and OASIS systems; vaccinations required per organization policy and annually thereafter; E-Verify participation applies
