Job Details:

• Location: Los Gatos, California
• Schedule: Days, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR Certification; valid and active license in the state of California
• Preferred Qualifications: Experience working in outpatient settings; previous successful assignment with the agency or organization (highly desired)
• Additional Information: Evaluates assigned patients, develops treatment plans based on clinical reasoning, completes medical record documentation accurately and timely, and ensures all care is provided under signed physician orders within insurance guidelines. Participates in student programs, pursues professional growth, adheres to state practice acts, maintains licensure, and contributes to organizational performance improvement initiatives.
