Job Summary:
Physical Therapist (PT) specializing in Home Health located in Kaneohe, Hawaii. The position involves providing specialized therapy services within a home health setting, with a focus on restoring and improving patients' physical, social, and mental functions. Candidates are required to bring their license on the first day of employment.

Job Details:

• Certifications: CPR, BLS (American Heart Association) required
• Education: Bachelors degree required; Masters degree preferred
• Licenses: Hawaii State Physical Therapist (PT) license required
• Experience: Minimum of 2 years of Physical Therapy experience required; 1 year of PT home health experience required
• Schedule: Days, 8 hours per shift, minimum 40 hours per week
• Travel experience: Preferred
• Clinical tools and documentation: Use of Homecare Home Base Electronic Medical Record (EMR) system and OASIS documentation required
• Responsibilities include planning and conducting treatment programs, performing evaluations, establishing individualized treatment and discharge plans, supervising assistants, and maintaining clinical documentation
• Must have own transportation, valid drivers license, and current auto insurance
• Mileage reimbursement at .43 cents per mile
• Coverage area includes Oahu Honolulu, Aiea, Pearl City, Mililani, Wahiawa.
• On-call and weekend duties: No on-call or weekend requirements
