Job Summary:
Greeley, Colorado Outpatient Physical Therapist. This role involves evaluating and developing treatment plans for outpatient patients, with a focus on helping them return to community activities. Candidates are required to bring their license on the first day. The position is scheduled to start on 06/29/2026, with an expectation of working 8 hours per day for a minimum of 40 hours per week.

Job Details:

• Specialty: Physical Therapist Outpatient (PT OP)
• Schedule: Days shift; 8 hours daily; minimum 40 hours per week
• Required Qualifications: Degree in Physical Therapy from an accredited school; active/applicable license for the state of Colorado; CPR certification; 6 months to 1+ year of outpatient clinical experience
• Preferred Qualifications: Has successfully completed an assignment with the agency previously (highly desired); has previously worked with the organization (nice to have)
• Additional Information: Evaluates patients, develops and modifies treatment plans, and documents clinical care in compliance with regulatory and organizational standards. Participates in student programs, pursues ongoing professional development, and adheres to state practice acts and licensing requirements. Demonstrates behaviors aligned with organizational core values including safety, quality, teamwork, and professionalism.
