Description
Job Summary: Physical Therapist (Home Health) Portland, Oregon
The Physical Therapist will provide home health services to adult and geriatric patients in the East Portland area and surrounding communities. The role supports a 1:1 patient ratio model and includes productivity expectations aligned with standard point targets. Candidates are required to bring their license on the first day.
Job Details:
- Schedule: Days, 8 hours per shift, with a minimum of 40 hours per week
- Required Qualifications: Basic Life Support (BLS); 1 year of skilled home health experience
- Preferred Qualifications: Oasis experience
- Additional Information:
- Home health caseload may include visits to up to 6 patients per day, with 30 productivity points standard
- Pending license is accepted
- First-timers are accepted
- On-call is not required
- Floating between like units is not required
- Block scheduling is not available
- Weekend requirement is not required
- Patient charting includes Electronic Health Record (Epic) and home health tools on a laptop and a phone app
- Business casual attire is required (no jeans); scrubs are allowed but not required
