Job Summary: Ada, Oklahoma Physical Therapist Assistant (SNF)
The organization is seeking a Physical Therapist Assistant to support rehabilitative, skilled, and medically necessary treatment interventions for patients with physical functioning disorders. Responsibilities include administering rehabilitation procedures, documenting clinical information in an electronic documentation system, and communicating patient progress and treatment outcomes to a supervising Physical Therapist. Candidates are required to bring their license on the first day.
Job Details
- Schedule: Days, 8 hours per shift, minimum 40 hours per week; no weekends; no call.
- Required Qualifications: Active Physical Therapist Assistant (PTA) license (when applicable); active Occupational Therapy (OT) license at the time of submission; 1+ years of PTA experience; long-term care (LTC) or rehabilitation (rehab) experience; resume; skills checklist completed within 1 year; 01 reference at time of submission.
- Preferred Qualifications: New graduates may be considered if they have Level II experience and not only Level I observation experience.
- Responsibilities:
- Utilize evidence-based practice to support clinical interventions under the supervision and direction of a Physical Therapist.
- Provide rehabilitative treatment interventions, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.
- Assess patient progress and communicate with the supervising Physical Therapist to adjust treatments and update clinical records per regulatory and clinical practice requirements.
- Report effectiveness of treatment outcomes to the supervising Physical Therapist.
- Provide consultation and counseling to patients and families/caregivers, as well as other service providers related to physical disorders.
- Communicate effectively with the health team regarding patient progress, barriers, and treatment plans.
- Provide input to support discharge documentation of services provided per regulatory and clinical requirements.
- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to organization policy.
- Accurately record treatment minutes for services provided per regulatory and clinical requirements.
- Participate in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
- Participate in facility and organization required trainings, in-services, and conferences.
- Work with the supervising Physical Therapist to obtain any necessary durable medical equipment (DME) or supplies per regulatory and clinical practice requirements.
- Supervise and support students under the guidance of the Physical Therapist.
- Adhere to APTA standards and applicable state practice acts, code of conduct, and ethics during professional practice as a Physical Therapist Assistant.
- Additional Information: Active state licensure is required when applicable.
