Description
Job Summary: Physical Therapist, Outpatient (PT OP) Wheat Ridge, Colorado
The organization is seeking an Outpatient Physical Therapist to evaluate assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction. The role supports functional treatment programming aimed at progressing patients toward discharge and community reintegration. Candidates are required to bring their license on the first day.

Schedule: Days shift, 8 shift hours, minimum 40 hours per week

Required Qualifications: Degree in Physical Therapy from an Accredited School; CPR Certification; Active/applicable license for the State where work will be performed; completion of a 0.0 years experience requirement as stated in the job order

Preferred Qualifications: 6 months to 1+ year of outpatient clinical experience; successful completion of an assignment with the organization previously; successful completion of an assignment with the organization previously; 6-month duration desired

Responsibilities: Completes comprehensive evaluations of assigned patients and uses clinical reasoning to develop treatment plans and goals focused on discharge within an established length of stay; completes medical record documentation using approved formats and in compliance with applicable guidelines and clinic policies; ensures care is provided under signed, valid physician orders and within insurance approval as necessary; participates in the student program as assigned, providing appropriate supervision and learning objectives; creates an individualized professional development plan and pursues professional growth through continuing education, self-study, and professional activities; adheres to discipline-specific State Practice Act and maintains professional licensure in good standing, including required continuing education; performs duties ethically in accordance with discipline-specific professional codes of conduct; follows organizational performance improvement methodology and supports action plans
