Job Summary
Physical Therapist (Home Health) Lebanon, Pennsylvania
The Physical Therapist (Home Health) will provide rehabilitation services in an outpatient-style home health setting, supporting adult and geriatric patient care needs across a schedule of 8-hour days Monday through Friday. The role includes clinical documentation responsibilities and may include care for COVID-19 patients. Candidates are required to bring their license on the first day.
Job Details
- Schedule: 8-hour days, Monday through Friday; minimum 40 hours per week.
- Float: Float: Float rotation may be required.
- Required Qualifications:
- Physical Therapist (Home Health) experience (minimum 1 year)
- Charting experience: EPIC
- Ability to document using OASIS (preferred)
- Certifications: Basic Life Support (BLS) and/or American Red Cross (ARC) accepted
- Vaccinations: COVID-19 vaccine required (medical/religious exemptions and declinations allowed) and flu vaccine required (medical/religious exemptions only)
- Preferred Qualifications:
- OASIS documentation abilities
- Additional Information:
- Travel/support may be needed within surrounding areas as part of assignment coverage.
- Tobacco- and fragrance-free environment.
- Supporting documentation must be uploaded for both work hours and mileage (if mileage is required for reimbursement).
