Job Summary
Occupational Therapist (OT), Home Health Portland, Oregon. The organization is seeking an Occupational Therapist to provide skilled home health services across the West Portland area and surrounding communities. Responsibilities include delivering therapy focused on functional goals and patient independence, with documentation completed using EPIC and remote tools. Candidates are required to bring their license on the first day.

Job Details

• Schedule: 8-hour Days shift; 40 hours per week minimum.
• Required Qualifications: Occupational Therapy Home Health experience (minimum: 1 year of skilled home health experience); Oregon license; Basic Life Support (BLS) certification; ability to provide skilled therapy supporting Activities of Daily Living (ADLs) and the stated home health clinical needs; familiarity with OASIS documentation experience is required (OASIS experience is referenced as required for this role); must have reliable transportation (as confirmed in submission).
• Preferred Qualifications: National Patient Safety Goals / Core Measures knowledge.
• Additional Information:
  • Home health field work serving adult and geriatric patient populations, including diagnoses such as dementia, Parkinsons, ALS, MS, and orthopedic/post-stroke needs.
  • Documentation uses EPIC remote client for home health on a laptop and Rover (phone app), and OASIS documentation requirements apply.
  • Expected to be able to travel across the assigned service area as scheduled and grouped by clients to minimize drive and windshield time; occasional maximum radius of roughly 60 miles per day may apply.
  • Attire guidance: business casual (no jeans); scrubs allowed but not required (no specific color).
  • Vaccination requirements include primary series COVID vaccination and one booster; Oasis experience is required.
