Job Summary:
The organization is seeking a Physical Therapist (PT) specializing in Home Health care to work in the Seaside, Oregon area. This role involves providing skilled therapy services to patients in their homes, with a focus on promoting recovery and independence. Job Details:

• Schedule: 8-hour days, starting from 09/08/2026, with a minimum of 40 hours per week
• Required Qualifications: Valid state license, at least 1 year of skilled home health experience, BCLS certification, and a compact license if applicable
• Preferred Qualifications: Home health experience preferred; experience with outpatient clinics, rehabilitation hospitals, skilled nursing facilities, and outpatient settings is beneficial
• Certifications: BCLS (Basic Cardiac Life Support)
• Additional Information:
  • The traveler is expected to have their own transportation, with occasional drives within a maximum radius of approximately 60 miles per day.
  • No on-call duties required.
  • The facility cannot accommodate 4x10s or 3x12s schedules.
  • Candidates must be willing to work on a block schedule and commit to working three out of five designated holidays.
  • RTO requests must be made during submittal and are limited to a maximum of 7 days to avoid delays.
  • Candidates should respond to the question regarding previous employment at Providence facilities if applicable.
  • Travelers may be expected to float between nearby sister facilities; refusal to float may lead to termination or loss of guaranteed hours.
  • The assignment will utilize any required modules during pre-start and orientation, with time recorded accordingly.
