Job Summary:
A Home Health Physical Therapist (PT) position is available in Seaside, Oregon. This role involves providing skilled home health services with an emphasis on patient mobility, rehabilitation, and education. The ideal candidate will bring at least one year of Skilled Home Health experience and must bring their license on the first day. Candidates should be prepared to work eight-hour days and meet the specified clinical and operational requirements.

Job Details:

• Location: Seaside, Oregon

• Minimum Hours Per Week: 40 hours
• Required Qualifications: One year of Skilled Home Health experience; current Basic Life Support (BLS) certification; Oregon state license; reliable transportation.
• Preferred Qualifications: Home health experience preferred.
• Certifications: BLS (Basic Life Support)
• Schedule: 8 hours per day, no weekend requirement, no on-call required, guaranteed up to one call-off per pay period.
• Additional Information:
  • The system groups clients efficiently to minimize drive time, with an occasional maximum radius of approximately 60 miles per day.
  • Traveler is expected to have their own transportation and meet specific vaccination requirements, including COVID-19 vaccination with booster.
  • Experience with Oasis documentation is required.
  • Candidates must provide two references covering at least one year within the last three years.
  • No facility pass or pass removal; no pass pass pass pick up sub/rent.
