Job Summary
Physical Therapist Outpatient (PT OP) Pasadena, CA. The organization is seeking an Outpatient Physical Therapist to evaluate assigned patients, develop and implement treatment plans, and support progress toward discharge goals aligned with patient needs and physician direction. Candidates are required to bring their license on the first day.
Job Details
- Schedule: Days shift; 10 shift hours; minimum 40 hours per week.
- Required Qualifications: Degree in Physical Therapy from an accredited school; CPR certification; 6 months to 1+ year of outpatient clinical experience; outpatient experience; must have an active/applicable California license for the state where work will be performed; CA license number already.
- Preferred Qualifications: Prior successful completion of an assignment with the organization previously; prior successful completion of an assignment with the organization previously is nice to have.
- Responsibilities: Conducts comprehensive patient evaluations and uses clinical reasoning to develop treatment plan goals focused on progressing patients toward discharge within an established length of stay. Completes timely and accurate medical record documentation, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats in accordance with applicable guidelines and clinic policies. Provides care under signed, valid physician orders and within insurance approval as indicated and necessary for clinical care. Participates in student programming as assigned, providing appropriate supervision and support for learning objectives and student evaluations. Creates an individualized professional development plan and pursues professional development through continuing education, self-study, and professional activities. Maintains compliance with discipline-specific State Practice Act and other regulatory requirements, including ongoing continuing education and ethical professional conduct. Supports performance improvement initiatives by following action plans and offering feedback through staff meetings, focus groups, or direct supervisor collaboration. Demonstrates behaviors consistent with the organizations mission, core values, and patient and employee safety focus.
