Job Summary: Physical Therapist (Outpatient) Santa Monica, California
The outpatient Physical Therapist is responsible for conducting comprehensive patient evaluations, developing and implementing individualized treatment plans, and documenting care in accordance with clinical and regulatory requirements. The role supports patient progress toward discharge goals and includes delegation and direction of discipline-specific therapy services for assigned patients. Candidates are required to bring their license on the first day.

Schedule: Days shift, 8 hours per shift, minimum 40 hours per week.

Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; active Basic Life Support (BLS) / CPR Certification (as applicable); no new graduate; active/applicable license for the state where work will be performed.

Preferred Qualifications: Prior successful completion of an assignment with the organization previously; prior successful completion of an assignment with the healthcare provider previously.

Responsibilities:
- Completes a comprehensive evaluation of assigned patients and uses clinical reasoning to develop treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
- Implements and modifies treatment plans based on patient needs, goals, and physician direction; supports functional treatment programs to promote return to community activities.
- Completes medical record documentation using approved formats, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines and facility policies.
- Ensures care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care.
- Participates in student programming as assigned, providing supervision and supporting development of learning objectives and student evaluations.
- Develops individualized professional development plans with clinical management and pursues ongoing professional growth through continuing education, self-study, and participation in professional activities.
- Adheres to the discipline-specific State Practice Act and other regulatory agencies; maintains professional licensure in good standing and completes all state-mandated continuing education requirements.
- Performs duties ethically in accordance with the discipline-specific professional code of conduct.
- Supports organizational performance improvement initiatives and follows through with action plans; provides feedback through staff meetings, focus groups, or direct supervisor channels.
- Upholds facility compliance expectations, confidentiality practices, and professional codes of conduct.
- Maintains patient and employee safety focus, including safe patient handling, materials handling, infectious disease precautions, and slips/trips/falls awareness.
