Description
Occupational Therapist (OT) Outpatient East Chicago, IN
An Occupational Therapist (Outpatient) provides patient-focused occupational therapy services by planning, organizing, and implementing individualized treatment plans in alignment with applicable federal, state, and local standards and guidelines. The role supports an interdisciplinary approach to care and contributes to discharge planning, documentation, and quality improvement efforts. Candidates are required to bring their license on the first day.
Job Details
- Schedule: Days shift; 8 hours per shift, with a minimum of 40 hours per week
- Required Qualifications: Basic Life Support (BLS) certification (required prior to the first shift); a valid, current state license; minimum of 2 years of experience
- Preferred Qualifications: None provided
- Additional Information:
- Assesses patients levels of functioning upon physician orders and completes initial evaluation documentation according to department policy and procedures
- Plans and implements appropriate occupational therapy treatment based on assessment results and patient/family goals
- Participates in the interdisciplinary team process, including team conferences and discharge planning (placement, education, and adaptive equipment as appropriate)
- Completes proper documentation including daily progress notes within defined timeframes and maintains an independent treatment program and/or caregiver training recommendations as appropriate
- Supervises Certified Occupational Therapy Assistants and supports supervision of OT and COTA students; assists with new staff orientation and provides coverage for scheduled absences
- Demonstrates patient and employee safety awareness, including safe patient handling, materials handling, infectious disease precautions, and prevention of slips/trips/falls
- Observes patient confidentiality and maintains a customer service attitude toward patients, visitors, and staff
