Job Summary: Los Angeles, CA Physical Therapist Outpatient. The organization is seeking a Physical Therapist to evaluate and treat assigned patients in an outpatient setting, developing individualized treatment plans aligned with patient goals and physician direction. This role includes managing clinical documentation, contributing to patient education, and supporting professional development while maintaining compliance with all applicable standards. Candidates are required to bring their license on the first day.

Job Details:

• Schedule: Days shift; 8 shift hours, minimum 40 hours per week, flexible scheduling.
• Required Qualifications: Degree in Physical Therapy from an accredited school; CPR Certification; active/applicable licensure for the State where work will be performed; 6 months to 1+ year of outpatient clinical experience.
• Preferred Qualifications: Prior experience successfully completed with the organization; prior experience successfully completed previously with the healthcare provider.
• Responsibilities:
  • Complete comprehensive evaluations of assigned patients and use clinical reasoning to develop treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
  • Provide and coordinate discipline-specific therapy services for assigned patients, including delegation and direction as applicable.
  • Ensure documentation is completed in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats in accordance with state, federal, and intermediary guidelines and clinic policies.
  • Ensure care is provided under signed, valid physician orders and, where required, within insurance approval for clinical care.
  • Participate in student programs as assigned, providing a positive learning environment, appropriate supervision, and completion of student evaluations.
  • Create and follow individualized professional development plans, including pursuing continuing education and applying learned information to meet strategic objectives.
  • Adhere to discipline-specific State Practice Act and other regulatory agencies while maintaining professional licensure in good standing and completing required continuing education.
  • Uphold professional ethical standards and codes of conduct applicable to the role.
  • Follow organizational performance improvement methodology and contribute feedback through staff meetings, focus groups, or direct supervisory channels.
  • Demonstrate behaviors consistent with organizational mission and core values, including patient and employee safety expectations.
