Job Summary
Physical Therapist Assistant (SNF) Worcester, MA. The position supports patients with physical functioning disorders by delivering rehabilitative, skilled, and medically necessary treatment under the supervision and direction of a Physical Therapist. Responsibilities include administering rehabilitation interventions, assessing progress, documenting care in an electronic documentation system, and communicating patient status and treatment needs with the interdisciplinary team. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Days, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Active Physical Therapist Assistant (PTA) license (as applicable); 1+ years of PTA experience; Long-Term Care (LTC) or Rehabilitation (Rehab) experience.
• Preferred Qualifications: None listed.
• Additional Information: Provides evidence-based practice to support clinical interventions; administers rehabilitation procedures such as manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive or assistive devices; assesses patient progress and communicates with the supervising Physical Therapist to adjust treatments; documents regulatory and clinically required information in the electronic documentation system; accurately records treatment minutes for services provided; participates in required meetings (including patient care conferences and utilization review meetings) and required trainings, in-services, and conferences; collaborates with the supervising Physical Therapist to obtain necessary durable medical equipment (DME) or supplies; supervises and supports students under the Physical Therapists guidance; adheres to the American Physical Therapy Association (APTA) standards and applicable state practice acts, code of conduct, and ethics.
