Job Details

• Schedule: Days, 8 hours shifts, minimum of 40 hours per week.
• Required Qualifications: Valid California Physical Therapist (PT) license, at least 1 year of experience as a Home Health Physical Therapist, valid driver's license, and auto insurance. BLS certification is preferred.
• Preferred Qualifications: Experience with adult and geriatric patient populations, proficiency with clinical documentation systems, and familiarity with home health care protocols.
• Float: Float rotation may be required.
• Additional Information: Position requires delivering direct patient care to diverse patient diagnoses including mobility, balance, and functional mobility assessments. The role involves collaboration with interdisciplinary teams, patient education, and adherence to agency policies and procedures. Orientation includes 40 hours of training prior to assignment start date. All required onboarding, documentation, and compliance deadlines must be met as specified.
