Job Summary: Physical Therapist, Home Health Lakeport, California
Physical Therapist (Home Health) position supports adult and geriatric patient care through in-home visits, with an emphasis on evaluation, treatment planning, and safety-focused mobility interventions. The role includes completing required compliance steps for onboarding. Candidates are required to bring their license on the first day.

Job Details:

Schedule: Days shift, 8 shift hours, with a minimum of 40 hours per week
Required Qualifications: California Physical Therapist (PT) license; at least 1 year of experience as a Home Health PT; valid California drivers license; Basic Life Support (BLS) preferred
Preferred Qualifications: BLS
Additional Information: Provides home health physical therapy services with an expected volume of approximately 45 patients per day (5 weighted visits per day). Interdisciplinary support includes Physical Therapy, Interpretation Services, and Social Services.
