Description

Job Summary: Physical Therapist Outpatient (PT OP) Boulder, CO
Outpatient Physical Therapist is responsible for completing comprehensive patient evaluations and developing, implementing, and modifying individualized treatment plans based on patient needs, goals, and physician direction. The role includes maintaining accurate and timely medical record documentation and ensuring care is delivered in accordance with valid physician orders. Candidates are required to bring their license on the first day.

Job Details:

  • Schedule: Days shift, 8 hours per shift, with a minimum of 40 hours per week.
  • Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR Certification; active/applicable license for the state where work will be performed.
  • Preferred Qualifications: Successful completion of an assignment with the organization previously; successful completion of an assignment previously with the organization; 6-month duration desired.
  • Completes comprehensive evaluations of assigned patients and uses clinical reasoning to develop treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
  • Completes medical record documentation in a clear, concise, accurate, and timely manner, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry in approved formats in compliance with applicable guidelines and clinic policies and procedures.
  • Ensures care is provided under signed, valid physician orders and, as indicated and necessary, within insurance approval for clinical care.
  • Participates in student program activities as assigned, providing a positive learning environment with appropriate supervision and student evaluation.
  • Creates an individualized professional development plan with clinical management, identifies and initiates professional growth, and pursues professional development through continuing education, self-study, and professional activities.
  • Adheres to the discipline-specific State Practice Act and other regulatory agencies, maintains professional licensure in good standing, and completes state-mandated continuing education requirements.
  • Performs in an ethical manner in accordance with discipline-specific professional codes of conduct.
  • Understands the organizations performance improvement methodology and follows through with action plans to improve the quality of services provided by this position.
  • Offers feedback through staff meetings, focus groups, or direct supervisor to maximize performance improvement outcomes.
