Description

Job Summary:
Home Health Physical Therapist (PT) in Portland, Oregon. This position involves providing skilled outpatient home health services within designated areas. The therapist will evaluate, Plan, and implement patient care plans according to established standards, collaborating with interdisciplinary teams to optimize patient recovery and independence.

Job Details:

Schedule: 8-hour days, minimum 40 hours per week, start date 08/10/2026
Required Qualifications: 1 year of Skilled Home Health experience, Oregon or Washington licensure, CPR/BLS certification, and valid license in the state of Oregon or Washington.
Preferred Qualifications: Experience with EPIC charting system and Oasis documentation.
Additional Information: The therapist is expected to have own transportation. Occasionally, driving within a maximum radius of approximately 60 miles per day may be required. The work environment involves outpatient home health care with interdisciplinary support. Travel within the state is expected, and the provider will be responsible for completing modules as required for compliance. Proper attire includes business casual or scrubs, and charting is conducted remotely via EPIC and Rover applications.
