Job Summary:
A Physical Therapist (PT) specializing in home health services is sought for a position based in Rock Hill, South Carolina. The role involves providing home health care to adult patients with chronic diseases, wound care, post-stroke recovery, and post-rehabilitation needs. Candidates are required to bring their license on the first day of work.

Job Details:

• Location: Rock Hill, South Carolina
• Schedule: 8-hour shifts, with a weekend rotation not typically including weekends unless specified as a weekend-only position (Friday/Saturday/Sunday)
• Required Qualifications: Certification as a Basic Life Support (BLS) provider, valid Driver's License, proof of Insurance, State license, and a minimum of 1 year of home health experience. Experience with Homecare Homebase electronic medical record (EMR) system, charting, OASIS-E assessment tool, and geriatric/Adult patient care is mandatory.
• Preferred Qualifications: Experience with above patient types, disease teaching, chronic disease management, gait speed, and gait belt use.
• Additional Information: Use of Samsung Note 9 phones, laptop, and other equipment provided. Orientation consists of 5 days of facility and unit training. A home health branch servicing York and Lancaster counties, with potential to float into Union or Mecklenburg counties but not likely. Support within the department is provided by departmental leadership.
• Support and technology: Homecare Homebase EMR, departmental leadership, and necessary equipment supplied.
• Travel: This is a weekend-only position; travel up to 100 miles may be required depending on caseload.
• RTO Requests: Require manager approval.
• Scrub Color: Light Grey Therapy.
• Additional notes: Holidays may be worked if desired; RTO requests must be approved by management.
