Description
Job Summary:
The Physical Therapist (PT) Home Health (HH) candidate will provide patient evaluations and therapeutic services in a home health setting in Dover, Delaware. The role involves delivering physical therapy in accordance with physicians' orders, developing treatment plans, and supervising other health personnel as appropriate. Candidates are required to bring their current license on the first day of assignment.
Job Details:
- Location: Dover, Delaware
- Shift Hours: Monday through Friday, 8:00 a.m. to 5:00 p.m.
- Minimum Hours Per Week: 40 hours
- Start Date: 07/06/2026
- Specialty: Physical Therapist Home Health (PT HH)
- Experience: 0 years required; 2-3 years of physical therapy experience preferred
- Certifications: Current CPR certification
- Licensing: Current, unencumbered license to practice physical therapy specific to Delaware
- Education: Successful completion of a physical therapy program approved by the American Physical Therapy Association or equivalent accrediting bodies
- Skills: Must be familiar with general use and functions of the computer, including internet, email, file navigation, online HR services, and online training programs
- EMR System: HCHB (Homecare Homebase)
- Experience in Home Health: Required, with experience in HCHB preferred
- Supervision & Responsibilities: Provide direct physical therapy, monitor patient progress, and participate in discharge planning and final assessments. Supervise no more than 2 full-time equivalent PTAs.
- Mileage: Variable weekly travel to patients' homes within Kent County, Delaware
- Productivity Expectations: Minimum of 30 points per week
- Reporting To: The designated Director of Operations / Clinical Manager, with approval from Angela Bivins for weekly time review
- Contract Duration: 13 weeks
