Job Summary:
The outpatient Physical Therapist (PT) specializing in outpatient therapy will provide clinical assessments and develop treatment plans for patients in a Longmont-based healthcare setting. This position involves performing evaluations, implementing therapy services, and ensuring patient progress within a dynamic outpatient environment. The role emphasizes patient-centered care and collaboration to facilitate optimal recovery and functional improvement.

Job Details:

• Position: Physical Therapist Outpatient (PT OP)
• Location: Longmont, Colorado
• Schedule: Days, 8-hour shifts, minimum of 40 hours per week
• Start Date: 08/24/2026
• Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR certification
• Preferred Qualifications: 6 months of outpatient clinical experience is highly desired
• Additional Information:
  Completes comprehensive patient evaluations and develops individualized treatment plans focused on progressing patients toward discharge within an established length of stay. Accurate and timely documentation of medical records, goal setting, and assessments are essential. The therapist participates in student programs, provides supervision, and pursues ongoing professional development through continuing education and professional activities. Adherence to state practice acts and regulatory standards, maintaining licensure, and practicing ethically are required. The role involves engaging in performance improvement initiatives and contributing to organizational quality efforts. The provider must demonstrate competencies as outlined by the organization and uphold safety standards in all clinical activities.
