Description
Job Summary:
The Physical Therapist (PT) specializing in Home Health in Rancho Cordova, California, provides in-home therapy services to a diverse patient population. This role involves treating patients with various diagnoses, including orthopedic, cardiovascular, pulmonary, oncological, neurological, geriatrics, and amputations. Candidates are required to bring their California Physical Therapist license on the first day of assignment. The position requires the ability to work independently in home environments with a schedule of Monday through Friday, 8:30 AM to 5:00 PM.
Job Details:
- Location: Rancho Cordova, California
- Shift Hours: 8 hours per day, Monday to Friday, 8:30 AM - 5:00 PM
- Minimum Hours Per Week: 40 hours
- Start Date: 07/21/2026
- Required Qualifications: California Physical Therapist license; BLS (AHA) certification; minimum 2 years of experience in a healthcare setting; home health experience including OASIS; proficiency with HomeCare Home Base (HCHB) electronic medical record system; valid drivers license and proof of automobile insurance; good oral and written communication skills; strong organizational skills.
- Preferred Qualifications: Community/home health experience.
- Additional Information:
- Mileage is billable per GSA guidelines, with personal timesheets signed by home health supervisors.
- Travel between housing and the first home health visit does not qualify for reimbursement.
- The PT supervises up to 2 PTA/COTAs.
- The role involves visiting 3 to 5 patients daily, including Start of Care visits and evaluations, with visits weighted to standard time frames for documentation and case management.
- The position involves covering various territories, mainly serving the Mercy San Juan Hospital in Sacramento, with the flexibility to cover other locations as needed.
- The orientation includes a 3-week training with online learning and precepting, moving to solo work by the end of the second or third week.
- No specific "must have" skills beyond the listed qualifications, but the ability to work in sometimes cluttered or challenging home environments is essential.
- Cancellation of up to 4 shifts per 13-week assignment by the hospital is permitted; holiday work is required but flexible if time off is requested.
