Description
Job Summary:
The organization is seeking a qualified Physical Therapist (PT) specializing in Home Health services in Rancho Cordova, California. This position involves providing in-home therapy treatments, including assessments and interventions for a variety of patient conditions. The role requires working within a collaborative team environment to deliver quality care to patients across designated territories.
Job Details:
- Schedule: Dayshift, Monday through Friday, 8:30 AM to 5:00 PM, with 40 hours per week
- Required Qualifications:
- Valid Physical Therapist (PT) license and current driver's license with proof of automobile insurance
- Certification in Basic Life Support (BLS) from the American Heart Association (AHA)
- Minimum of 2 years of experience in a healthcare setting, with community/home health experience preferred
- Experience with Homecare Home Base (HCHB) electronic medical record system and OASIS is preferred
- Must have a current California Physical Therapist license
- Preferred Qualifications:
- Home health experience including OASIS documentation
- Ability to work independently within varied home environments
- Good organizational, oral, and written communication skills
- Additional Information:
- Caseload typically includes 3 to 5 patients daily, with a mix of Start of Care visits and evaluations
- Work involves supervised therapy in homes, assisted living, senior apartments, and board-and-care homes
- No mileage expenses will be reimbursed; candidate should be comfortable with travel within designated territories that group patient locations to optimize efficiency
- Dress code requires business professional attire; scrubs are not provided
- Robust three-week orientation with online learning and in-field precepting is part of onboarding
- Solo work is expected by the end of the second or third week of orientation
- Weekend work is requested once a month but not required; holiday work may be scheduled with flexibility upon request
- Shift is managed by an RN Manager with support from the Regional Rehab Leader/Educator
- No "Must Have" skills beyond the required certifications and experience, but ability to work in diverse home environments is important
