Job Details:

• Travel Requirements: Occasional travel to central and northern regions; mileage reimbursed at government-approved rate
• Float: Float rotation may be required, with occasional on-call duties for low census hours; on-call hours count toward low census hours and may involve call-back pay
• Weekend Work: Rotation of one Saturday every 6-8 weeks with a weekday day off during the week
• Holidays: None
• Certifications: AHA or ARC Basic Life Support (BLS); licensure for Wisconsin required; valid drivers license with proof of car insurance; NPI required (applicant must initiate process if not yet obtained)
• Education: Bachelors Degree in Physical Therapy
• Experience: Home health experience with adult/geriatric patients, including orthopedic, neurological, and mobility diagnoses; minimum of 0 years required
• Equipment: Laptop for documentation, NetSmart EMR, cell phone for GPS and communication
• Additional Information: Contract length of 13 weeks with a start date of 07/20/2026; travel policy during flu season requires mask-wearing for unvaccinated individuals; failure to comply with vaccination policies may result in corrective action
