Job Details:

  • Serves four counties (Noble, Lagrange, Steuben, DeKalb), primarily rural with mid-sized towns
  • Facility uses a point system for patient visits, equivalent to 4-6 visits daily
  • Electronic Medical Record system: Homecare Homebase (HCHB)
  • Must perform Oasis Assessments and Start of Care cases
  • Cancellation policy allows hospital to cancel up to 4 shifts per 13-week assignment, or 3 shifts for shorter assignments
