Description
Job Summary:
Physical Therapist (PT) specializing in Skilled Nursing Facility (SNF) settings in Missouri. This role involves evaluating and treating patients with physical functioning disorders and wound care, developing comprehensive care plans, and supporting recovery through evidence-based interventions. The position requires collaborative communication with the healthcare team to ensure quality patient outcomes.
Job Details:
- Schedule: Day shift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: 1 year of physical therapy experience including Level II experience, licensed in Missouri, and experience in long-term care or rehabilitation settings
- Preferred Qualifications: Experience supervising and supporting physical therapist assistants, rehabilitation aides, and students; familiarity with electronic documentation systems and regulatory standards for therapy documentation
- Additional Information: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and wound care, following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practices to support clinical interventions, administers procedures such as manual techniques, ambulation, therapeutic exercises, modalities, supportive/devices, wound care, and more. Monitors progress, adjusts treatments, evaluates outcomes, and provides consultation to patients and care providers. Participates in patient care conferences, utilization reviews, and necessary training sessions. Supervisory responsibilities include supporting physical therapist assistants, rehabilitation aides, and/or students. Adheres to professional practice acts and ethical standards during service delivery.
