Description

Job Summary:
The organization is seeking a Physical Therapist (PT) specializing in Skilled Nursing Facility (SNF) settings in Oklahoma City. This role involves evaluating and treating patients with physical functioning disorders and providing wound care in compliance with regulatory and clinical practice standards. The position offers an opportunity to work closely with patients to develop individualized plans of care and support their recovery process.

Job Details:

  • Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
  • Required Qualifications: Oklahoma State License; 1 year of physical therapy experience with Level II clinical experience preferred; LTC or rehabilitation experience; ability to effectively screen, evaluate, and rehabilitate patients; familiarity with standardized assessments and evidence-based practice; and strong communication skills.
  • Preferred Qualifications: Experience supervising and supporting physical therapist assistants, rehabilitation aides, and students; adherence to the American Physical Therapy Association (APTA) and State practice acts, ethics, and code of conduct.
  • Additional Information: Effectively screens, evaluates, and develops treatment plans for patients with physical functioning disorders; administers various therapeutic procedures, including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care; communicates progress and treatment plans with the health care team; documents all services in accordance with regulatory requirements; participates in patient care conferences, utilization review meetings, and mandatory trainings; collects necessary durable medical equipment (DME) and supplies; and supports clinical supervision of relevant staff.
