Job Details

• Position: Physical Therapist (SNF)
• Location: Ada, Oklahoma
• Schedule: Days shifts, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Valid Oklahoma license; 1 year of PT experience including long-term care or rehab; Degree in Physical Therapy from an accredited program; ability to obtain any necessary durable medical equipment or supplies; Must meet state licensure and regulatory requirements.
• Preferred Qualifications: Experience working in skilled nursing or rehabilitation settings; familiarity with evidence-based practice and standardized clinical assessments.
• Additional Information: Effectively screens, evaluates, and develops treatment plans for patients with physical disorders or wound care needs. Administers procedures, monitors progress, and adjusts treatments accordingly. Provides consultation and counseling to patients, families, and caregivers. Documents all clinical activities in electronic records and participates in team meetings, trainings, and conferences. Supervises and supports rehabilitation staff, adhering to practice acts, codes of conduct, and ethics.
