Job Summary:
Physical Therapist specializing in Skilled Nursing Facilities (SNF) is needed in Worcester, Massachusetts. This role involves evaluating and treating patients with physical functioning disorders and wound care, developing individualized care plans, and supporting patient recovery through evidence-based interventions. The therapist will also provide consultation, document care services, and coordinate with other healthcare team members to optimize patient outcomes.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Certifications, MA State License, 1 year of physical therapy experience. New graduates with Level II experience and not just observation experience are eligible. Experience in long-term care or rehabilitation settings is required.
• Preferred Qualifications: None specified.
• Additional Information: Effective screening, evaluation, and rehabilitation of patients with physical disorders, including wound care and therapeutic procedures. Supervision of physical therapist assistants, rehabilitation aides, and students. adherence to the American Physical Therapy Association (APTA) and Massachusetts practice acts, code of conduct, and ethics during professional practice. Participation in meetings, trainings, and conferences as required.
