Job Summary:
A Physical Therapist specializing in Skilled Nursing Facility (SNF) settings is sought in Oklahoma. This role involves evaluating and treating patients with physical functioning disorders, implementing evidence-based interventions, and developing comprehensive care plans. The position offers an opportunity to support patient recovery and improve quality of life through clinical expertise and collaboration within a healthcare team.
Job Details:
- Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week
- Required Qualifications: Oklahoma state license, 1 year of Physical Therapist experience, including Level II experience in a long-term care or rehabilitation setting; new graduates with appropriate experience are considered
- Preferred Qualifications: Experience in skilled nursing facility or rehabilitation environments
- Additional Information: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care, following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practices to support clinical interventions. Administers procedures including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care. Assesses patient progress and adjusts treatment plans accordingly. Provides consultation, counseling, and collaborates with other healthcare providers regarding patient outcomes. Documents all interventions and progresses in the electronic documentation system, maintaining compliance with regulatory standards. Participates in team meetings, trainings, and conferences. Supports supervision of therapy assistants, aides, or students as applicable. Ensures adherence to the American Physical Therapy Association (APTA) guidelines and state practice acts.
