Job Summary:
Physical Therapist (PT) with specialization in Skilled Nursing Facility (SNF) practicing in Oklahoma. This role involves effectively screening, evaluating, and rehabilitating patients with physical functioning disorders and/or wound care needs. The position focuses on developing appropriate plans of care, administering procedures, and assessing patient progress to enhance recovery outcomes, all while adhering to regulatory and clinical practice standards.

Job Details:

• Schedule: 8-hour shift, minimum of 40 hours per week, starting 08/24/2026
• Required Qualifications: Licensure as a Physical Therapist in Oklahoma; 1 year of physical therapy experience preferred; new graduates with Level II experience are acceptable; experience in long-term care (LTC) or rehabilitation settings
• Preferred Qualifications: Not specified
• Float: Float rotation may be required
• Additional Information: The ideal candidate will provide consultation and counseling to patients and their families, effectively communicate with the healthcare team about patient progress, and maintain thorough documentation in accordance with clinical and regulatory requirements. Participation in meetings, trainings, and supervision of rehab aides or students is expected. Candidates must adhere to the American Physical Therapy Association (APTA) and state practice acts, ethics, and conduct standards.
