Job Summary:
Physical Therapist (PT) specializing in Home Health in Charlotte, North Carolina. This position involves providing patient care within a home health setting, managing chronic diseases, wound care, IV therapy, and other related services. The role requires a dedicated healthcare professional capable of working independently and adhering to facility standards.
Job Details:
- Schedule: Dayshift, 8 hours per day, with a minimum of 40 hours per week.
- Required Qualifications: One (1) year of experience as a Physical Therapist in home health. Valid Driver's License and proof of Insurance are required. State Licensure and BLS certification are mandatory. Proficiency with Homecare Homebase electronic health record system (EMR), charting experience, and assessment skills are required.
- Preferred Qualifications: Experience with geriatric and adult patient populations, chronic disease management, and wound care.
- Additional Information: Must have skills in charting with Homecare Homebase, IV therapy, pleurx, wound vac, and disease teaching. The position includes a five-day orientation. Scrub color is grey, and parking is free. No on-call duties unless requested. Weekend rotation is not required.
- Support within the Department: Departmental leadership available.
- Technology/Equipment: Samsung Note 9 phones provided.
