Job Summary:
The location is in Lexington, Kentucky. This is a home health Physical Therapist (PT) position focused on patient care in a home setting. The role involves providing patient evaluations and ongoing therapy services according to clinical guidelines, with key responsibilities centered around home health visits and care planning.

Job Details:

• Schedule: 8-hour shifts, Monday through Friday, with rotating weekends. The standard shift hours are 8:00 AM to 5:00 PM, with a minimum of 40 hours per week.
• Required Qualifications: Valid license to practice as a Physical Therapist, BLS (American Heart Association) certification, at least 1 year of experience in home health setting, and familiarity with Homecare Homebase (HCHB) charting system.
• Preferred Qualifications: No specific preferences listed.
• Responsibilities: Conduct start of care assessments, recertifications, evaluations, and ongoing patient visits. Cover CHF, orthopedic, and pneumonia diagnoses. Provide patient care in the home health environment with a focus on therapy services.
• Additional Information: No clinical involvement outside of therapy services. A clinical manager or educator is available 24/7 for emergency support or during regular business hours. Two weeks of orientation with a preceptor is provided, including a few visits with a work coach. Scrubs or business casual attire are acceptable.
• Submission Requirements: License to practice must be in hand upon submission.
