Job Details:

• Schedule: Days shift, 8 hours per shift, minimum of 40 hours per week.
• Required Qualifications: Vermont Physical Therapist licensure; home health experience and OASIS experience are required as this candidate will case manage; EMR experience (McKesson/Netsmart/Homecare Homebase) strongly preferred.

• Float: Float rotation may be required.
