Description
Job Summary:
Physical Therapist Home Health in Portland, Oregon. This role involves providing skilled physical therapy services in a home health setting, supporting patient recovery and independence. The position is suitable for candidates with at least one year of relevant experience and offers an opportunity to work in a dynamic clinical environment within the community.
Job Details:
- Schedule: 8-hour days, minimum of 40 hours per week.
- Required Qualifications: Valid physical therapy license, at least 1 year of skilled home health experience, BCLS certification, reliable transportation, COVID-19 vaccination series in addition to one booster, and experience with community health environments.
- Preferred Qualifications: Experience with EPIC electronic charting system and familiarity with home health care protocols.
- Additional Information: The traveler is expected to have their own transportation. The role may involve visiting up to 6 patients per day with drive times averaging around 60 miles per day. The position requires adherence to facility policies, including dress code and charting requirements using EPIC and Rover. The expected start date is 08/24/2026. Candidates should be prepared to work within a community setting, supporting patient mobility, post-surgical recovery, and chronic condition management. References covering at least one year are required. All travelers are expected to float between facilities within the state, and refusal to float may impact employment terms.
