Job Summary:
Inpatient Pharmacist position in Charleston, West Virginia. The role involves providing pharmaceutical services within the hospital, following safety protocols, and ensuring the accuracy and proper dispensing of medications. The pharmacist will participate in medication management, patient education, and quality assurance initiatives.

Job Details:

• Schedule: 8-hour shifts, minimum 40 hours per week
• Required Qualifications: Pharmacist licensure, BLS certification if required by state, EPIC (Electronic Medical Record) experience, and a minimum of 2 years of recent hospital experience
• Preferred Qualifications: Experience with medication reconciliation, antimicrobial stewardship, and pharmacokinetic dosing management/monitoring, along with clinical knowledge for infectious diseases and anticoagulation management
• Additional Information: Maintains adequate drug supply, oversees inventory and disposal of outdated medications, ensures compliance with all federal and state pharmacy laws, and participates in quality assurance and medication therapy monitoring. Provides patient discharge counseling and education, maintains confidentiality, and supports accreditation and continuous quality improvement efforts. Performs other duties as assigned, including staff training and documentation.
