Job Summary:
Home Health Physical Therapist (PT) specializing in home health care is sought for assignments in the Hood River, Oregon area. This role involves providing outpatient physical therapy services to adult and geriatric patients in a home health setting, with a focus on delivering high-quality care and maintaining optimal patient outcomes. The position begins as soon as possible and offers a consistent eight-hour workday within a supportive home health environment.

Job Details:

• Shift: 8-hour days, scheduled for the start date of 09/07/2026.
• Certifications and Education: Oregon and Washington state licensure required; BLS certification mandated.
• Experience: At least 1 year of skilled home health experience; prior Oasis documentation experience preferred.
• Patient Ratios: Up to 6 patient visits per day; patient ratios and visit expectations are consistent with home health standards.
• Schedule: 40 hours per week, working days only, with no weekend or on-call requirements.
• Additional Qualifications: Experience with electronic charting using systems such as Epic and Rover; reliable transportation required due to travel between patient homes within the designated service radius.
• Travel and Location: The candidate must be within a 50-75 mile radius of Hood River, Oregon, with occasional driving up to approximately 60 miles per day.
• Special Requirements: COVID-19 vaccination (primary series and one booster) required; Oasis certification is necessary.
• Other Information: The traveler is expected to have their own transportation, and the employer will provide minimal grouping to optimize drive time. The facility requires adherence to safety protocols, proper documentation, and the ability to perform comprehensive assessments and treatments.
