Job Summary:
The organization is seeking a Physical Therapist (PT) specializing in Home Health in Everett, Washington. This role involves delivering patient-centered care within a home health setting, focusing on geriatric populations. The candidate will be responsible for managing a caseload, documenting accurately, and adhering to clinical protocols to support patient recovery and well-being.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Two years of recent experience in home health care, valid licensure in-hand for Washington State, proficiency in the HCHB (Homecare Homebase) charting system, and OASIS experience for staff completing Start of Care.
• Preferred Qualifications: Recent experience with geriatric patient populations, and familiarity with outpatient and rehabilitation hospital environments.
• Additional Information:
  • Travel up to a 60-mile radius from the designated hub or office; local candidates accepted.
  • Must be flexible with territory coverage.
  • Cell phone not required; iPad provided.
  • Mileage will be reimbursed at the IRS standard rate.
  • Travel pairs are accepted, but each traveler must have their own transportation and be assigned separate territories.
  • Monday orientation is mandatory for all travelers.
  • RTO (Requested Time Off) must be submitted at the time of application.
  • On-call rotation may be required; details to be discussed during interview.
  • Former employees must be separated for at least six months prior to returning as travelers and require HR approval.
  • Professional attire: scrubs of any color; jeans are not permitted.
