Job Summary:
Physical Therapist (Home Health) in HILO, Hawaii. This position involves providing rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). The therapist will perform evaluations, develop and implement personalized treatment plans, and deliver evidence-based therapeutic interventions to enhance residents' function and quality of life. The role requires collaboration with an interdisciplinary team and supervisory responsibilities when applicable.

Job Details:

• Location: HILO, Hawaii
• Schedule: Rotating shifts. Typically scheduled for five days per week, with the possibility of including one weekend day.
• Required Qualifications: Current State of Hawaii license in good standing appropriate to the discipline, Basic Life Support (BLS) certification, and minimum of two (2) years of experience in the SNF/ICF setting preferred. Less than two (2) years of experience is acceptable.
• Preferred Qualifications: Experience in rehabilitation services within SNF/ICF settings, familiarity with electronic medical records such as Net Health Rehab, and ability to supervise therapy assistants.
• Additional Information: Candidate must arrive the Thursday before the start date to attend employee health appointment on Friday. No facility, hospital, clinic, organization, or employer names are included to maintain anonymity.
