Physical Therapist Home Health - Baton Rouge, Louisiana. This temporary assignment involves providing skilled physical therapy services in a home health setting, adhering to physician plans of care, state regulations, and agency policies. The role requires collaboration with patients, families, and health care team members to promote optimal patient outcomes.
Job Details
- Schedule: Days, 8 hours per day, with a minimum of 40 hours per week, starting on 08/10/2026.
- Required Qualifications: Current unencumbered license to practice physical therapy specific to Louisiana. Successful completion of a physical therapy program from an accredited institution approved by the American Physical Therapy Association or equivalent. 0-3 years of physical therapy experience preferred. Current CPR certification. Familiarity with computer systems, including internet, email, and online HR services.
- Preferred Qualifications: Experience in home health settings. Knowledge of HomeCare HomeBase or PointCare EMR systems.
- Additional Information: This assignment is located within a 50-mile radius from Baton Rouge, Louisiana, with travel estimated at 150-250 miles per week. The role involves providing care at least 30 patient units per week and may include periodic on-site assessments, supervision of therapy assistants, and participation in team meetings. No weekend rotation or on-call duties are required.
