Job Summary:
Physical Therapist Outpatient in Conifer, Colorado. This role involves evaluating and developing treatment plans for outpatient patients, focusing on helping individuals regain functional independence. The position requires delivering quality therapeutic services in accordance with clinical standards and organizational values.

Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Degree in Physical Therapy from an accredited school, licensure valid for the state of Colorado, CPR certification, 6 months to 1+ year of outpatient clinical experience, and the ability to start on 08/17/2026
• Preferred Qualifications: Successful prior assignment with the agency or organization, or previous experience with Select Medical is highly desired and nice to have
• Additional Information: Responsible for completing comprehensive patient evaluations, developing individualized treatment plans, updating medical records accurately and timely, and ensuring care is provided under signed physician orders. Actively participates in student programs, pursues continued professional development, and adheres to regulatory standards and professional code of conduct. Demonstrates commitment to safety, quality improvement, and organizational values throughout clinical duties.
