Job Summary:
Home Health Physical Therapist position available in Minneapolis, Minnesota. This role involves assessing clients' needs, developing and implementing therapeutic plans, and collaborating with interdisciplinary teams to promote patient independence and well-being. The position requires providing direct care in a post-acute setting with a focus on home health services.

Job Details:

• Schedule: 8-hour days; 40 hours per week, starting on 08/17/2026
• Required Qualifications: Completion of an accredited physical therapy program; national registration and licensure for the state of practice; minimum of 1 year of clinical experience; ability to communicate and comprehend effectively in English; must be at least 18 years of age.
• Preferred Qualifications: Prior work experience as a therapist in a clinical setting helpful.
• Certifications: BLS Certification required.
• Supervisory Responsibilities: May include supervision of assigned employees, including hiring, training, performance appraisals, and fostering a positive work environment.
• Additional Information: This position involves compliance with all relevant laws, regulations, policies, and procedures. It may require travel as per organizational needs. Physical demands include lifting up to 50 lbs and active engagement in patient assessment and therapy activities.
