Physical Therapist (PT) Home Health in Vermont. Job Details:

• Location: 600 Granger Road, Barre, VT 05641, United States
• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Vermont Physical Therapist licensure; Home Health experience and OASIS experience are essential, as this candidate will case manage; McKesson/Netsmart Electronic Medical Record (EMR) experience is strongly preferred; no requested time off.
• Preferred Qualifications: Experience with therapeutic measurement techniques including Range of Motion (ROM), strength, and balance tests; ability to utilize various therapeutic modalities.
• Additional Information: The position emphasizes collaborative patient assessments and treatment planning to maximize patient outcomes and independence.
