Physical Therapist Home Health in the location area. This position involves providing home health therapy services to geriatric patients within a designated territory, focusing on post-operative care, rehabilitation, and management of chronic conditions. The role requires flexibility, timely communication, and adherence to established clinical protocols.
Job Details

- Shift Hours: 8 hours per day
- Minimum Hours Per Week: 40 hours
- Required Qualifications: Valid license in hand, minimum of 1 year of relevant experience, recent experience with at least 30 points weekly, proficiency in Homecare Homebase (HCHB), BLS certification, active driver's license, and personal transportation.
- Responsibilities include managing patient care for geriatric populations, completing all charting by 9 a.m. the following morning, and participating in on-call rotation.
- Schedule: Days, 8 hours per day, with a guaranteed minimum of 40 hours weekly.
- Additional Information: All charting must be completed by 9 a.m. the following morning. Cell phone use is not permitted; iPads are provided. Mileage is reimbursed at the IRS standard rate.
- Preferred Qualifications: Experience with OASIS for staff completing Start of Care, familiarity with hospice or home health operations, and experience with modules during orientation.
- Submission Requirements: Valid license in hand, active driver's license, and RTO (Requested Time Off) submitted at the time of application.
