Job Summary:
Physical Therapist specializing in Skilled Nursing Facilities in North Easton, Massachusetts. This role involves effectively screening, evaluating, and rehabilitating patients with physical functioning disorders and wound care needs. The position emphasizes developing appropriate care plans, utilizing evidence-based assessments, administering various therapeutic procedures, and documenting services in compliance with regulatory standards. Experience level is flexible, including new graduates, provided there is Level II experience.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: 1 year of physical therapy experience; Long-Term Care (LTC) or rehabilitation experience preferred; Massachusetts State License required; New graduates considered if they have Level II experience and not just Level I observation experience.
• Preferred Qualifications: Experience in Skilled Nursing Facilities (SNF).
• Additional Information: The physical therapist will assess patient progress, modify treatment plans, consult with patients and their families, and coordinate with other healthcare team members. Responsibilities include supervising physical therapist assistants, rehabilitation aides, or students, and participating in meetings, trainings, and documentation following all clinical and regulatory requirements.
