Job Summary:
The healthcare provider is seeking a Physical Therapist specializing in Skilled Nursing Facility (SNF) settings in Lowell, Massachusetts. This role involves evaluating and rehabilitating patients with physical functioning disorders and wound care needs, developing appropriate care plans, and supporting patient recovery through evidence-based interventions.

Job Details:

• Position: Physical Therapist SNF
• Location: Lowell, Massachusetts
• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
• Required Qualifications: Valid Massachusetts State License; 1 year of physical therapy experience including long-term care or rehabilitation settings; new graduates with Level II experience are welcome to apply
• Preferred Qualifications: Experience working in SNF or rehabilitation settings, familiarity with regulatory standards and clinical documentation
• Additional Information: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care; utilizes standardized assessments and evidence-based practices; administers procedures such as manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive device use, and wound care; evaluates patient progress and adjusts treatment plans accordingly; documents all clinical activities per regulatory and clinical practice standards; participates in team meetings, training sessions, and required conferences; supervises and supports rehabilitation aides and students; adheres to professional practice acts, codes of conduct, and ethics.
