Job Summary:
Physical Therapist Outpatient (PT OP) in Norwalk, Connecticut. This outpatient position involves treating ortho and potential neuro patients within a physical medicine and rehabilitation setting. The role includes performing physical therapy evaluations, developing treatment plans, and providing patient and family education.

Job Details:

• Location: 34 Maple Street, Norwalk, CT 06856
• Schedule: Days, 8 hours shifts, minimum of 40 hours per week
• Required Qualifications: Connecticut Physical Therapist (PT) license, Basic Life Support (BLS) certification, at least 1 year of experience working with patients with neurological conditions
• Preferred Qualifications: Vestibular therapy experience; no on-call or floating responsibilities
• Responsibilities:
  • Treat outpatient physical therapy patients with orthopedic and neurological conditions
  • Conduct physical therapy evaluations and implement treatment plans
  • Provide patient and family education and training
  • Complete documentation timely per department guidelines
  • Initiate discharge planning and perform other duties as assigned
