Description
Job Summary:
Physical Therapist Home Health (PT HH) West Portland, OR and surrounding areas. This position involves providing skilled physical therapy services within a home health setting, supporting adult and geriatric patients. The role requires an active Oregon license and prefers candidates with at least one year of home health experience.
Job Details:
- Location: West Portland, OR and surrounding areas
- Schedule: 8-hour days, minimum of 40 hours per week, starting on 09/08/2026 and ending on 12/08/2026
- Required Qualifications: Valid Oregon state license, BLS certification, at least 1 year of skilled home health experience
- Preferred Qualifications: Experience with EPIC charting system and Oasis documentation, wound care specialization, prior community hospital experience
- Patient Ratio: up to 6 patients per day with a 1:1 ratio, potentially visiting homes within a maximum radius of 60 miles per day
- Other Details: Candidates are expected to have their own transportation; occasional drive time may be up to 60-90 minutes daily. Must be vaccinated against COVID-19 (primary series plus one booster). The role involves charting using EPIC and Rover applications remotely.
- Certifications Required: BLS; manual therapy, wound care, and hand therapy skills are preferred.
- Special Requirements: Open to first-time travelers with relevant experience. No weekend rotations required. No on-call duties.
